Background:
While New Hampshire has some of the highest rates of substance misuse by youth and young adults in the Northeast, we are by no means unique in dealing with substance misuse and addiction issues. These behavioral issues are some of the most pressing issues facing every state in our Nation, so it is no surprise that healthcare providers are being asked to play a significant role in the public health response. Harmful drinking and risky drug use are common and costly, and there is a direct connection between substance use and oral health. In January 2018, the American Dental Association president encouraged dental professionals to recognize risky substance use and addiction and be prepared to counsel patients and refer them to treatment.
Screening, Brief Intervention, and Referral to Treatment (SBIRT) is a cost effective public health approach that enables early identification of patients who may engage in problematic substance misuse, and opportunities for prevention. SBIRT implementation, in multiple settings to identify problematic alcohol and other drug use, is prioritized in the NH Governor’s Commission on Alcohol and Other Drugs Action Plan for January 2019-December 2021: Expanding Our Response.SBIRT is utilized across private practices, community health centers, and hospital-based primary care and social service settings in NH, as well as dental practices.
Oral health providers play an important role in addressing this issue since they have ongoing relationships with individuals and some people may see a dentist more often than they see a primary healthcare professional. Oral health providers are uniquely positioned to identify individuals with harmful substance use and help them get the resources they need.
AIM:
Increase knowledge and capacity of oral health providers to conduct universal screening, provide a brief intervention as needed and arrange follow-up and treatment for excessive substance use in their practice.
Training Objectives:
This introductory training aims to increase oral health providers’ competence and confidence across the practice to routinely screen patients on their substance use and provide an appropriate Brief Intervention using motivational interviewing techniques.

- Review brief intervention techniques that utilize motivational interviewing skills. Instructors will provide a menu of motivational interviewing tools.

Lea LaFave, PhD, RN
JSI Research & Training Institute, Inc. - Consultant
Lea Ayers LaFave, PhD, RN, a senior consultant at Community Health Institute/JSI Research and Training Institute, Inc., provides consultation and technical assistance in the areas of quality improvement (QI) and action learning, and evaluation with a particular focus on community capacity building and systems change in regional, national and international arenas. Dr. LaFave embraces an improvement model that fosters empowerment, collaboration and shared learning through action learning collaboratives focused on implementing evidence-based practices. She has developed and managed QI action learning collaboratives across a range of topics and practice areas, including improving hypertension and diabetes management in rural health clinics; improving asthma management in community health centers and school-based clinics; and supporting the development of QI capacity at the local implementing agency level for the New Hampshire Maternal, Infant, and Early Childhood Home Visiting (MIECHV) Program.
Dr. LaFave led the NH Youth S•BI•RT Screen and Intervene Initiative, providing technical assistance to 23 pediatric-behavioral health partner clinical sites including hospital-based and FQHC-based practices to embed S•BI•RT (Screening, Brief Intervention, and Referral to Treatment) as standard practice for 12–22 year olds. The Initiative screened over 15,000 youth over three years, and produced a document outlining lessons learned throughout the implementation process. Lessons learned from this initiative informed the development of the Playbook to Implement Youth S•BI•RT in Primary Care Settings in NH, which has been adapted for Juvenile Diversion Programs in NH. Dr. LaFave also led a collaborative process to develop the S•BI•RT Implementation Playbook for Perinatal Providers. She currently provides technical assistance to support S•BI•RT implementation in dental practices.
Dr. LaFave holds a BA in Psychology from Smith College, a Master of Science in Nursing from Yale University, and a PhD in Nursing from the University of Massachusetts.

Martha Bradley, MS

Martha Bradley provides consultation on a wide variety of public health projects in NH. Her areas of expertise include health communication strategies, training and program development and implementation, and community mobilization. For nearly twenty years while working for JSI, during which she has created, developed and delivered multiple learning activities on a range of public health topics. Employing a learner-centered process to identify content, she has worked with various sectors to develop lessons, easy-to-read tip sheets, activities, provider and patient toolkits, communication and decision making tools and policies to inform and educate diverse audiences Ms. Bradley has worked on several projects aimed at expanding these professional’s ability to engage and motivate their patients/clients reduce risks and engage in health affirming behaviors. Whether they are child care professionals seeking to serve healthier foods or increase physical activity to the children they serve, primary care providers supporting patients to reduce risky substance use, or community partners prepare and respond to public health emergencies, M.s Bradley employs a client-centered approach with shared decision making.

Amy Moffett, BA
JSI Research & Training Institute, Inc. - Project Manager
Amy Moffett, B.A., is a Project Manager for JSI-NH. Her background as a Quality Assurance Tester, Project Manager and certified ScrumMaster (CSM) led to work in business requirement development, quality assurance testing, and user support. Her experience in Agile Methodologies and a wide variety of project management tools has led to successful oversight of new functionalities and business processes at organizations of all sizes. In addition, she has overseen an IT Help Desk which included the development of an online Knowledge Base, training, the creation of user manuals, and mentoring new staff to allow them to build upon their skillsets in their current roles as project team members. Ms. Moffett works on multiple initiatives at JSI-NH, with experience in SBIRT, staff equity and advanced facilitation. She has a Bachelor of Arts degree and Minor in Social Work from the University of New Hampshire and is an active volunteer and public speaker for NAMI NH.
